IGF2R (the cation-independent mannose-6-phosphate receptor, CI-MPR) is a multifunctional transmembrane receptor that couples lysosomal enzyme trafficking to growth control and tissue homeostasis (PMID:8806828, PMID:31748500). Through a hydrophobic binding pocket in domain 11, modulated by domain 13, it directly captures extracellular IGF-II via the IGF-II residues Phe19 and Leu53, and routes it for lysosomal degradation, thereby limiting IGF1R-mediated growth signaling (PMID:18046459); mouse genetics establishes this clearance function as the basis of its growth-regulatory and tumor-suppressor activity, since Igf2r-null overgrowth and lethality are fully rescued by loss of Igf2 or Igf1r, and Igf2r overexpression suppresses IGF-II-driven mammary tumors (PMID:8806828, PMID:16452186). In parallel, its M6P-binding activity directs newly synthesized cathepsins from the Golgi to lysosomes; loss of this function causes cathepsin missorting, lysosomal and autophagic dysfunction, and increased pericellular cathepsin activity that promotes invasion (PMID:31748500, PMID:22521359).
