Loss-of-function mutations in CTSC cause the allelic Mendelian disorders Papillon-Lefèvre syndrome and Haim-Munk syndrome, characterized by palmoplantar keratosis, severe periodontitis, and impaired neutrophil serine protease activity and NET formation (PMID:10581027, PMID:10662807, PMID:34932608). In cancer, tumor-secreted CTSC activates neutrophil PR3 to drive an IL-1β–NF-κB inflammatory cascade and NET-dependent degradation of thrombospondin-1, promoting metastatic colonization, a pathway pharmacologically targetable by DPP-1 inhibitors such as brensocatib (PMID:33450198, PMID:32897034).
