PODXL (podocalyxin-like protein) is a cell-surface sialomucin that controls cell adhesion, migration, and surface receptor presentation across stem cell, cancer, and neuronal contexts (PMID:18818395, PMID:31759250). In pluripotent and progenitor cells it functions as the carrier of pluripotency-associated post-translational epitopes (TRA-1-60, TRA-1-81, GCTM-2/gp200), and its abundance is set during reprogramming by KLF4; antibody engagement of its surface epitope is acutely cytotoxic, marking it as a defining surface determinant of these cells (PMID:12504081, PMID:26886504). On mesenchymal stem cell progenitors PODXL acts as an anti-adhesive that prevents cell aggregation, with high-PODXL cells showing greater clonogenicity and migratory capacity (PMID:18818395). In cancer, PODXL drives migration, invasion, and metastasis: it binds directly to the complement receptor C5aR, stabilizing it and recruiting it to the plasma membrane so that C5a stimulation triggers epithelial-mesenchymal transition, and its loss abolishes metastatic capacity in vivo (PMID:31759250). PODXL expression is constrained post-transcriptionally by multiple 3'UTR-targeting miRNAs and activated transcriptionally by TCF3, with downstream inflammatory signaling proceeding through the P38 MAPK pathway (PMID:26892887, PMID:34422820, PMID:36096977). A homozygous loss-of-function frameshift mutation in PODXL segregates with autosomal-recessive juvenile Parkinsonism, and mutant PODXL alters neurite branching and length, linking the protein to neuronal morphology (PMID:26864383).
