PAQR7 (mPRα) is a membrane progesterone receptor that mediates rapid, nongenomic progesterone signaling across multiple cell types including vascular smooth muscle, neural cells, and ovarian granulosa cells. Structurally homologous to adiponectin receptors, it adopts a 7-transmembrane topology with an intracellular N-terminus and a zinc-containing ligand-binding pocket in which residues Q206 and V146 are critical for progesterone binding (PMID:35189329). Upon ligand engagement, PAQR7 couples to inhibitory G proteins (Gi) to suppress cAMP, activate ERK and PI3K-Akt signaling, reduce RhoA/ROCK activity, increase SERCA2-mediated sarcoplasmic reticulum Ca²⁺ uptake, and decrease myosin light chain phosphorylation, collectively mediating vascular smooth muscle relaxation (PMID:29428395, PMID:31416049, PMID:33427050). PAQR7 also promotes neuroprotection via PI3K-Akt and MAPK pathways, drives Schwann cell-like differentiation and BDNF release through Src and PI3K-Akt signaling, regulates oocyte maturation, and is required for normal ovarian follicle development and granulosa cell survival through BCL-2/BAX/CASPASE-3 signaling, as demonstrated by increased apoptosis and reduced fertility in Paqr7 knockout mice (PMID:36967764, PMID:33930460, PMID:38317224, PMID:27554928).
