Decorin (DCN) is a secreted extracellular matrix proteoglycan that governs collagen fibril organization and acts as a multifunctional signaling modulator restraining fibrosis and tumor progression (PMID:36534610). Frameshift mutations in DCN cause congenital stromal corneal dystrophy, where loss of normal protein disrupts lamellar collagen spacing and fibril formation in the corneal stroma, establishing its structural role in matrix assembly (PMID:36534610); DCN similarly promotes ordered collagen fibril organization during tendon healing (PMID:41313163). As a signaling molecule, DCN functions as an antagonist of EGFR and downregulates receptor levels in tumor cells (PMID:18520035), and its restoration in carcinoma cells and stromal fibroblasts suppresses proliferation, invasion, and metastasis while inducing P21 and E-cadherin and reversing cancer-associated fibroblast activation through STAT3/AUF1 signaling (PMID:26547587, PMID:38667295). DCN suppresses myofibroblast activation and pro-fibrotic gene expression, consistent with an anti-fibrotic function. In injured and stressed tissues DCN engages cytoprotective and immune programs: it is released from ferroptotic cells via secretory autophagy and lysosomal exocytosis and binds the macrophage receptor AGER to trigger NF-kB-dependent pro-inflammatory cytokine production as a damage-associated molecular pattern (PMID:34964698), and it promotes autophagy-mediated clearance of intracellular H. pylori (PMID:39145305). DCN expression is controlled at multiple levels: transcriptionally activated by EGR3 binding to its promoter (PMID:40660758) and by AhR recruitment that is blocked by +58CpG methylation (PMID:24424784), and post-translationally destabilized by OTUB1-mediated ubiquitination and proteasomal degradation (PMID:22279542).
