ALB encodes a multifunctional plasma protein whose biological roles span transport, redox modulation, and tumor suppression, and whose loss has distinct systemic metabolic consequences (PMID:38876053, PMID:42126373). Its immunomodulatory and antioxidant function depends on the redox state of the Cys34 thiol: reduced-thiol albumin suppresses pro-inflammatory and pro-fibrotic macrophage remodeling via MAPK signaling and induces SIRT3, which deacetylates SOD2 to limit mitochondrial ROS and attenuate liver fibrosis, an activity absent in oxidized albumin (PMID:38876053). As a transport protein, functional albumin is required for circulating calcium and vitamin D handling, and missense gain-of-function variants at Arg242 (p.Arg242His) alter thyroid hormone-binding affinity to cause familial dysalbuminemic hyperthyroxinemia, while homozygous or compound heterozygous loss-of-function mutations cause congenital analbuminemia (PMID:27834068, PMID:34142517, PMID:38736368, PMID:31250585). Monogenic ALB loss-of-function additionally produces a distinct form of severe LDL-C elevation, diverging mechanistically from polygenic hypoalbuminemia (PMID:42126373). In cancer, ALB acts as a tumor suppressor: in clear cell renal cell carcinoma its overexpression activates endoplasmic reticulum stress, suppresses EMT, and reprograms glycolysis through negative regulation of SPARC, while its hepatic mRNA expression is destabilized by WTAP-mediated m6A modification in osteosarcoma (PMID:36988233, PMID:36690262, PMID:39862995, PMID:41610717).
